 I understand and agree that I am fully responsible for any damage and/or harm caused by my pup(s) while my pup(s) is/are at Chicago’s Active Tails LLC. 

I understand and agree that in admitting my pup(s), Chicago’s Active Tails LLC has fully relied upon my judgment that my pup(s) is/are in good health and have not harmed or shown aggressive behavior towards any person or any other pup(s).

I fully understand that due to the way that pup(s) interact with each other by nature, minor irritations, cuts, and scratches can occur even when they are carefully supervised at all times. While my pup(s) is/are under the care and custody of Chicago’s Active Tails LLC, if I am unreachable in the unfortunate event of an emergency, I hereby authorize Chicago’s Active Tails LLC, and/or representatives to immediately seek veterinary care for my pup(s). I fully understand that all costs in connection with veterinary, medical, or other treatment shall be my responsibility if not covered by or exceed insurance coverage.

I hereby release and agree to save and hold harmless, Chicago’s Active Tails LLC, its employees, and its members from any liability, claims, suits, actions, loss, injury, or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my pup(s) may sustain or which may be caused in any way by my pup(s). I specifically, without limitation, agree to fully indemnify Chicago’s Active Tails LLC for any such liability, claims, suits, actions, losses, injury, and/or damage. 

Although we carefully screen all applicants, occasionally we discover that this is not an appropriate environment for every pup. Chicago’s Active Tails LLC reserves the right to permanently remove a pup from any provided service at any time. In addition, Chicago’s Active Tails LLC reserves the right to disallow at its discretion certain breeds to participate in group activities. 

Chicago’s Active Tails LLC has a specific requirement when purchasing a service package. For Walk packages, there is a minimum booking requirement of three or more walks per week. For Visit and Daycare packages, there is also a minimum requirement of three or more bookings per week. All packages expire two months from the day of purchase.

Chicago’s Active Tails LLC has a specific requirement that all services be paid in full prior to, during, or the very next day the service has been successfully completed.
An invoice will be sent via email. 

I certify that I have read and understand all the regulations set forth herein and that I have fully read and understand this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions, and statements of this agreement, and confirm the truthfulness of the contents of the Application form completed by me.
